How to Enroll
Step 1: Pick a plan
CalPERS offers Health Net HMO health plan options to Basic members. Our HMO plans use a provider network that connects you to local, trusted, high-quality providers and hospitals. HMO plans are ideal if you prefer a primary doctor to coordinate all your medical care at predictable costs. You simply choose a primary care physician (PCP) from our network of providers. Your PCP will oversee all your health care-related services, including referrals and authorizations to specialists. Our HMO plans offer affordability, predictability and convenience:
- $0 medical deductibles.
- Fixed copayments for most services.
- Your doctor directs most of your health care needs.
- You don't need a referral to see an OB/GYN in the medical group. Just make your appointment.
- No claim form filing (except emergency out-of-network care).
- Rewards to help you improve your health and well-being.
- You can access outpatient behavioral health care from an in-network provider without a referral from your PCP.
- Acupuncture and chiropractic coverage through American Specialty Health Plans, Inc. (ASH).

- A tailored network of quality providers.
- Cross-border benefits using the SIMNSA network in Northern Mexico.
- $0 copayment to use Teladoc Health.
- $50 gift card for taking the RealAge® test, plus an additional $25 gift card for sharing the results with your PCP.
- $25 gift card for completing three Health Coaching calls plus an evaluation call.
- $25 gift card for completing Eat Right Now-Mindful Eating and Craving to Quit® (or any other two Sharecare offerings).
- $25 incentive for using Welvie® prior to a surgery.

Step 2: Choose a doctor
When enrolling in an HMO plan, please make sure your primary care physician (PCP) or participating physician group (PPG) is part of your plan's provider network. Each eligible member of your family can choose a different PCP and a different PPG to meet their personal needs. A PPG is sometimes referred to as a "medical group."
ProviderSearch tips
- Before using our ProviderSearch tool, log in to your Health Net member account.
- Make sure your location is within the Health Net CalPERS contracted area.
- Select the plan – Salud HMO y Más (CalPERS members only) – from the drop down filter to narrow your results to CalPERS contracted providers.
- Write down your doctor's name, medical group name, plus the 6-digit Provider ID number. (You'll need to put this information in the "Notes" section of the my CalPERS site).
Step 3: Enroll in Health Net today!
Once you've compared the various health care coverage options that are available to you during Open Enrollment, contact your health benefits officer or personnel office for required forms and documentation.
